As explained at the Ukrainian Embassy in Poland, this can happen for two reasons:
- program failure;
- you left Poland more than once a month.
If there is such a problem, you should contact the local authorities (zhond) to restore it. If this is not possible there, you will be sent to the Border Guard (Straż Graniczna), you can contact the regional offices for foreigners (Urząd Wojewódzki, Wydział Spraw Cudzoziemców), or the Office for Foreigners in Warsaw.
